Potty training regression - help!

My puppy was doing SO well with potty training and now suddenly having accidents everywhere. Nothing has changed in our routine.

She was fully house trained for about 2 weeks and now we're back to square one. Is this normal?

Should I go back to treating every successful potty outside like she's a brand new puppy?
